Jan Voi C. Mojica

In all my roles within the healthcare and hospice industry, I have often felt helpless in providing patients with the timely care they desperately needed. Hospice patients and their families face urgent and unique challenges, and the systems in place frequently fall short in addressing these needs efficiently. My mission is to solve the recurring problems that families and healthcare providers encounter with hospice care. I aim to restore faith in the hospice industry by developing personal systems that enhance efficiency, reduce overhead costs, and improve patient care. I want to be an asset to hospitals, skilled nursing facilities, board and cares, and assisted living facilities, ensuring that every hospice patient receives the best possible support during their final journey. Additionally, I am committed to reducing hospital readmissions, benefiting both hospitals and patients.
 

With a background as an Emergency Medical Technician, I learned the importance of swift, efficient systems. This foundation has guided me throughout my extensive career in the hospice industry. I have held various roles, each time developing personal systems to perform my job more effectively and to address the specific needs of patients and their families.

My journey in hospice care began in 2014 as a patient care secretary, where I created efficient systems for after-hours patient care and Continuous Care scheduling. Over the years, I advanced through roles such as float secretary, Community Liaison, sales representative, Director of Marketing, and Operations and Brand Manager. In each position, I focused on building strong relationships with referral sources, coordinating patient care, and improving operational efficiency. My efforts have consistently led to providing referrals to partners they had never received referrals from before and obtaining recurring referrals from our referral sources.

We collaborate with Assisted Living Facilities (ALFs) and Board and Cares to pilot innovative programs that generate strong support and referrals. These referrals enable us to craft targeted marketing strategies that connect hospice providers with key healthcare partners, including hospitals, skilled nursing facilities, doctors, and clinics.

We leverage social media to build brand awareness and provide educational content on hospice care and the progression of terminal illnesses. By keeping the community well-informed and engaged, we ensure that hospice providers can operate more efficiently and build stronger relationships within the healthcare community.
